Jennifer is the President and CEO of Cornerstone Whole Healthcare Organization and provides operational expertise to primary and specialty care practices throughout the State of Idaho interested in implementing integrated behavioral health programs. Her work allows providers to manage the behavioral health population and helps the patient achieve the best health and quality of life possible by preventing chronic disease, stabilizing current chronic conditions, and preventing acceleration to higher risk with higher healthcare costs.
Leo Carney III, DO, FAAFP
Dr. Carney, Chief Medical Officer at Cornerstone Whole Healthcare Organization, leads clinical operations for the Idaho Health Neighborhood Center. A distinguished healthcare leader with over 20 years of experience, he previously served as Director of Clinical Programs at Headquarters Marine Corps, transforming their Primary Care model. His visionary leadership also implemented process improvements across 57 military medical facilities and developed a groundbreaking governance model. Dr. Carney’s expertise extends to managing large programs, as evidenced by his role as Director of Primary Care and Mental Health at the Bureau of Medicine and Surgery. He holds a Doctor of Osteopathic Medicine degree and is board-certified in Family Medicine.
Jacob Wilson, LCSW
Jake is the Chief Operations Officer of Cornerstone Whole Healthcare Organization and provides day-to-day operational oversight for the company. Jake received his Bachelor’s degree in Social Work from Boise State University and his Master’s degree in Social Work from the University of Nevada, Reno. For the past two decades, he has worked in community mental health, private practice, inpatient and emergency room, integrated behavioral health, healthcare administration, and various other areas of the field. Prior to coming to C-WHO, Jake provided clinical and operational oversight for all IBH programs as well as a specialty behavioral health clinic in a large healthcare system. Over the years Jake has developed a passion for improving healthcare access to rural and underserved populations through programmatic development and community partnerships. Jake lives in McCall, Idaho with his wife and two children.
Linda Cardwell
Linda Cardwell is the Chief Marketing Officer for Cornerstone Whole Healthcare Organization. She obtained her Bachelor of Science in Marketing from Azusa Pacific University with an emphasis in providing marketing expertise to non-profits. Her background includes project management, content development, partner management, marketing automation, and digital marketing. She has over 10 years of marketing experience and is passionate about utilizing her expertise to help nonprofits move their missions forward. At C-WHO Linda has created brand consistency to establish a higher level of professionalism and customer loyalty. She has provided effective marketing materials (flyers, brochures, social media campaigns, and website design) to increase communication and overall promotion of programs and networks. She has implemented and managed a learning platform for asynchronous training for healthcare professionals and has assisted in conference and event management. Linda not only brings this skillset and expertise to C-WHO but to all of C-WHO’s customers as a tailored service to increase their effectiveness in their communities.
Ross Novick, JD
Ross Novick is the Chief Financial Management Officer with Cornerstone Whole Healthcare Organization, INC (C-WHO). Ross obtained his Bachelor of Arts degree from the University of Colorado Boulder and a Juris Doctorate from Golden Gate University. He has extensive background in finance and management working in NW Arkansas and California. He has a passion for helping non-profits achieve their goals. When Ross is not working, he loves to run and mountain bike on the incredible trails of NW Arkansas.
Anthony Leon
Anthony Leon is the Multimedia Manager in the Marketing Division at Cornerstone Whole Healthcare Org. He received his Bachelor of Science in Interdisciplinary Entrepreneurship from Florida Gulf Coast University. He has a background in social media marketing, digital audio, and music composition.
Scott Malm PA-C
Scott Malm, Executive Director of Ambulatory Services at Idaho Health Neighborhood Center, Nampa. With over 26 years as a Physician Assistant, he brings expertise in Family Medicine, Urgent Care, Orthopedics, and more, alongside military experience as a Certified Surgical Technologist and awards such as Soldier of the Year Idaho Army National Guard.
Alanna Lilly, PA-C
A compassionate Physician Assistant specializing in Family Medicine, Alanna brings over 8.5 years of experience to the Treasure Valley community. With a master’s degree from the University of South Alabama and a bachelor’s degree in Biology, she’s deeply rooted in the local community. Beyond medicine, Alanna finds joy in fantasy novels, outdoor adventures, and savoring culinary delights. Her unwavering dedication to healthcare and zest for life enriches our community.
Sara Barrett
Sara is a Behavioral Health Consultant and a Licensed Clinical Social Worker at the Idaho Neighborhood Health Center. She received her Master’s of Social Work at Colorado State University, and up until recently, she had been living and serving the populations of rural, Western Colorado. Sara brings nearly ten years of experience in integrated behavioral health with her to Idaho and has a passion for providing high-quality behavioral health services with as few barriers as possible. She has worked with every age and stage of the lifespan and enjoys the challenge of being a “jack of all trades” behavioral health consultant in family medicine. Sara lives in Boise with her husband and two children.
Becky Hardy
With 29 years of experience, Becky is an accomplished Medical Assistant whose expertise spans chronic care management, endocrinology, women’s health, integrative medicine, and rheumatology. She is recognized for her unwavering commitment to patient care, excellent clinical abilities, and skill in supporting both patients and healthcare providers in a variety of medical environments.
Qasim Modaqiq
Qasim, a physician from Afghanistan, moved to the U.S. in September 2021. With a Bachelor of Health Science and medical education in Kazakhstan, he is currently a Physician Extender at Idaho Health Neighborhood Center in Nampa, aiding clinicians and assisting patients. Previously, he worked as a phlebotomist with the American Red Cross. Qasim loves Idaho’s natural beauty, has made friends, and is dedicated to becoming a U.S. physician, currently preparing for the USMLE.

Desiree “Dezzi” Medina is an experienced manager and healthcare professional with over 25 years of leadership in various roles, renowned for her strategic insights and compassionate approach. She has made a profound impact in the medical field through her commitment to excellence and patient care, holding roles from frontline caregiving to administrative leadership. Dezzi’s dedication to continuous improvement is evident through her pursuit of leadership courses and certifications, reflecting her passion for fostering excellence and innovation in every endeavor.
Melissa Smith, MD
Melissa Smith, MD, is a board-certified psychiatrist with a diverse and accomplished medical background. Dr. Smith began her medical career in general surgery, with specialized training in trauma and critical care. She later transitioned to psychiatry, completing her residency at Emory University. Over the years, she has served as a teacher and supervisor for resident physicians and fellows, building a strong foundation in both psychopharmacology and complex medication management. Regardless of setting, her clinical philosophy remains the same: meet people where they are, listen with intention, and build trust—one conversation at a time.
